End of Life Offerings & Services

 End of Life (EOL) Consultative Care is a companion offering for palliative and hospice care clients who wish to consciously determine their final life journey. Dharma Transitions provides consistent, non-medical, nonjudgmental emotional support and consultative services from prognosis through bereavement.

As your EOL Consultant, I partner with you to advocate and ensure your desires are fulfilled.

My role is to hold space for you and your loved ones to be together and share in the experience of creating a purposeful and peaceful death, however long that takes. I facilitate the planning and completion of your goal to live life to the last moment as you want. In this journey, I work with you to develop your EOL plan and provide dedicated emotional support through that process, including addressing the ever-present grief that accompanies this life stage. I assist you in arranging your transition through death with love and intention. I manage the details to relieve you of the weight of the process, and allow you time to focus meaningfully on being with your family.

Intake & Introduction

Emotional Support & Companioning

 

Providing client-focused holistic, emotional support and guidance through your end of life experience; including such possibilities as comfort care, guided imagery, energy work, prayer or meditation, and caregiver respite support.

Legacy Development

 

Planning how you wish to be uniquely remembered: such as, creating a personal narrative, a memory project, letters to loved ones, a final wish fulfillment, or in some way that is meaningful to you and your circle.

Ensuring your final wishes are observed with Advanced Care Directives and appropriate legal services.

Funeral & Memorial Planning

 

Consulting on cremation and funeral options and preferences.

Organizing loose ends including the dispersement of personal goods.

Determining how and with whom you wish to spend your final time.

Honoring your dying process so that it feels intentional and complete.

Vigil & Bereavement Support

 

Companioning you through your final hours.

Performing an observance or ritual to acknowledge the completion of your embodied journey.

Providing psychopomp support to guide you through your transition from body to spirit.

Assisting your inner circle through the initial bereavement period to ensure they feel supported in their grief.
